At Quantum Hospice & Palliative Care, we are committed to serving our patients with exceptional care, providing comfort and encouragement in their time of distress, confusion or loss.
Upon admission, patients will receive care through one of our four levels of hospice care.
Basic level of care provided in your home, assisted living facility, nursing home, or wherever patients call home.
When medical needs cannot be managed in the home.
Medical management in the home for up to 24 hours a day when medically appropriate.
Temporary care provided for up to 5 consecutive days in nursing home, hospice, inpatient facility, or hospital so that the family member or friend who is the patient's caregiver can take some time off.
Hospice care is a specific type of medical care that involves serving patients in the final stages of life. Hospice care serves both patients and their loved ones during this challenging time. The focus of hospice is providing compassionate care with comfort and gentleness to support patients and their loved ones during the final stages of life.
Eligibility is determined through a physician's certification stating the patient has a terminal diagnosis.

We also provide palliative care. Palliative care is an interdisciplinary medical care giving approach aimed at optimizing quality of life and mitigating suffering among people with serious, complex and often terminal illnesses.
Any patients with a serious, long-lasting disease or life-threatening illness are eligible for palliative care at any stage of their illness.
Along with a specialized plan of care, each hospice patient is provided with the necessary medical equipment and supplies needed to efficiently and effectively receive care services. Example: hospital bed, wheelchair, toiletries, etc.
